Serious Injuries
In the event that a serious injury occurs in
your game, whether or not a penalty is called, it is important to document
what happened. For these purposes, please use the Official's Game Report
to outline the incident.

in the event of any dangerous conditions, broken equipment, or if any
arena equipment (shot
clock, nets, score clock) is missing at game time.
